Misc code cleanup

This PR attempts to fix as many miscellaneous formatting/syntax problems highlighted by code climate and landscape.io tests.

The commits will come rolling in, mainly so that the test suite can run several times to determine the improved scoring, so when I'm done I'll remove the pull-request-incomplete tag.
